You can download forms here and submit them to the Police Department in person, by fax to 503-543-2955, or in the mail at the address to the right.

Nuisance Complaint - You can submit nuisance complaints via this form, or by calling the non-emergency dispatch number at 503-397-1521. An officer will get back to you to investigate the complaint.

Service Comment Report- To file a service comment or complaint, you can download and complete the Service Comment Report. A supervisor will get back to you to discuss the matter.

Records Request- If you need a copy of a police report, fill out this form and bring it to the police department, or mail it in along with your payment. We will contact you when the record is ready to be picked up, or we can mail it to you. Some records are archived and require extra time to retreive, and cost more. Not all police reports are public record and some requests and information will not be released without prior approval of the City Attorney or District Attorney.

Vacation Check Request - If you will be leaving your home for an extended period, you can request a vacation check.

Alarm Permit Application- Scappoose City ordinance requires an alarm permit on any business or residence where there is an audible alarm, whether it is monitored or not. The annual fee for an alarm permit is $30 for residences (Age 62 and over are exempt) and $100 for businesses (no age exemption).

Citizens Academy Application- Citizens Academy begins in May. You may download an application and mail it back, or stop by the Police Department and fill one out. Spaces are limited.

Ride-along Request- To gain a better understanding of what our officers do and deal with on a daily basis, you can request a ride-along. Just fill out an application and upon approval, you will be contacted to schedule your ride.

Business Watch Membership- Business owners interested in reducing crime that affects their business can join Business Watch. Just provide your contact information, and we will contact you regarding future meetings and information that impacts your business. You can also join Business Watch by calling Lt.
